Get a Copy of Your Screening

To ask for a copy of a completed screening with the priority score, contact the Aging and Disability Resource Center (ADRC).  If a person believes the answers provided to the screening were not recorded right, he or she can contact the ADRC to request to update the screening, or he or she may ask for a Fair Hearing.
